Mucinous tumors account for 10–15% of all primary ovarian tumors. 1 Approximately 80% are benign and the remainder are borderline tumors, noninvasive carcinomas, and invasive carcinomas. 1 Although they generally occur in older women (mean ages 51–54 years), mucinous borderline tumors and carcinomas are more common in the first two decades than their serous counterparts. 2 Mucinous tumors
Borderline ovarian tumors (BOTs), or low malignant po-tential tumors, represent a rare and special tumor entity.1 They are epithelial tumors of the ovaries characterized by cellular proliferation and nuclear atypia but without an in-filtrative growth pattern.2 A subgroup of BOTs presents microinvasion, a histological feature characterized by the

Microinvasion (Required) A standardized quantitative criterion for distinguishing microinvasion from frankly invasive carcinoma within a borderline tumour has not been established, and varying definitions have been used in different studies, including 1 Borderline ovarian tumors are relatively uncommon with an incidence of 1.5–2.5 per 100,000 people per year.

With microinvasion: Foci of stromal invasion, measuring < 5 mm in the greatest dimension Degree of cytologic atypia is mild and similar to borderline tumor Areas of mucin extravasation with inflammatory response are not diagnostic of invasion serous borderline tumors and serous borderline tumors with non-invasive micropapillary pattern [1]. Eclipse E600 microscopeA particular aspect of typical or micropapillary serous borderline tumors is stromal microinvasion. The concept of microinvasion in serous borderline tumors has been recognized for nearly 50 years [2,3]. Therefore, borderline mucinous ovarian tumor with microinvasion is not reportable. Low malignant potential/borderline ovarian tumors are defined by the pathology of the primary tumor in the ovary, and microinvasion there, or invasion in implants does not change that diagnosis. Some studies conclude that serous borderline tumors with microinvasion have a similar prognosis to that of the normal serous borderline tumor [2, 12,19], and conserving the contralateral ovary and Nayar et al.

SINQ 20170043 is a similar question about an ovarian mucinous borderline tumor with microinvasion, but the answer seems to be specifically referencing mucinous tumors only. It is unclear if that SINQ could be applied to this case.

Microinvasion may be represented by a single focus or multiple
Although exploratory, our data suggest that BOTs with microinvasion present earlier relapses, but overall incidence of relapses and overall survival do not differ significantly from BOTs without microinvasion. Microinvasion was common but, in our series, did not appear to worsen the prognosis.
